
Here are the easy steps on how to secure an official digital copy of Vaccination Certificate issued by the Department of Health. You need to be fully vaccinated in order to proceed so be ready with your Vaccination Card. Database from the local government units are sent to the centralized database by the Department of Information, Communications and Technology (DICT) called VaxCertPH.
STEP 1. Log on to https://vaxcert.doh.gov.ph/. Click on the question “Are you traveling abroad?”

Select YES then click OK.

Click on “What is your vaccination status?”

Choose Fully Vaccinated then click OK. If you choose Partially Vaccinated, the processing will not proceed.

Click on “When did you receive your first dose?”

Choose “More than 48 hours” then click OK, otherwise, the processing will not proceed.

Click NEXT.

STEP 2. Input all Personal Information.


Input the dates and location of your 1st and 2nd doses. If you received Johnson &Johnson vaccine, tick the box at the top.




STEP 3. Review the details as displayed on the screen then click CONTINUE.

Type the 7 characters shown on the screen then click OK.

If you input the wrong data, “No Vaccination Record Found” will be displayed on screen.

Or if you’re data is correct but the screen still states NO VACCINATION RECORD FOUND, you can UPLOAD your vaccination card together with any Government ID.

STEP 4. If your record is found, this will be displayed. Click GENERATE VACCINATION CERTIFICATE.

Your digital Vaccination Certificate will be displayed on the screen. Click DOWNLOAD VACCINATION CERTIFICATE.

Congratulations! You’re done. Save this and have it printed and laminated.

If you don’t have internet access, you can visit City Hall where there is a registration booth for VaxCertPH..
